HCP / Home Care Provider/ Caregiver in Manchester and surrounding cities/towns.

HCP will receive a monthly tax-exempt (non-taxable) stipend.

HCP will receive an additional payment for 30 hours of the CPS program every week on top of their yearly tax -exempt income of $70000.

HCP will provide supervision and assistance in the comfort of their own home to a developmentally challenged individual to live a meaningful life.

RESPONSIBILITIES:

- Follow supervision requirements per the individual's Annual Service Agreement.

- Provide transportation to the individual as needed.

- Provide personal care and support daily living skills including, assisting the individual with increasing living skills to improve independence.

- Coordinate all medical and dental appointments and administer medication, as required.

- Support participation in community activities and other areas of interest.

- Complete and submit monthly documentation, as required by regulations.

- Maintain a safe and comfortable environment that meets all State and Fire regulations.

REQUIREMENTS:

Valid Driver's License, clean motor vehicle and criminal records, BEAS, DCYF clearance, reliable vehicle, comprehensive personal automobile coverage, and renters/homeowners insurance.
